no problem with Debian package ... you wrote: "I reinstalled the Debian
package, and it started working..." ... so there was some mistake with
your customized configuration 

i used old courier without any problems on Debian stable

Debian Etch is frozen - so no new changes except bugfixes ... i'm using 
Etch on my servers; but anyway courier is great package, so i used new
courier with authlib with stable debian without problems ...
