As I was typing this, I tried one more configuration with Kmail. I added
my IMAP account and forced kmail to use inbox. as the personal
namespace. I was able to see all my folders at the same level as Inbox,
but none of the folders have any items, even though I know there are
some. This doesn't work in Thunderbird.

Also a note. I installed Roundcube webmail to my server. It shows all
the directories on the same level, but I'm guessing it's probably just
figured it out, whereas my mail clients can't.

Anyway, does anyone have any ideas?

Try asking for help on the mail clients' relevant mailing lists, e.g. the kmail mailing list, the Thunderbird mailing list, etc. There's far more likely you'll find someone there with extensive knowledge of these clients' configuration tricks.

It is true that the IMAP protocol allows a properly-written IMAP mail client to automatically configure itself, like your "Roundcube webmail" appears to have done.
