Sorry, as that was almost a year ago, the exact solution escapes me. :-) Honestly though, I think it was simply a matter of upgrading. I'm running courier-0.42.2 now, and STARTTLS works great.
I'll copy the list in case someone else can provide more help...

I can use imapd-ssl and pop3d-ssl fine, but when I try "STARTTLS" on port 25, it answers "220 OK" and drops connection. Full text of transaction:
