Solved it!

I apologize for polluting the list with what turned out to be a simple
mistake on my part.  The actual problem was that esmtpd.authpam was
missing a 'd' in the filename!  

I am still curious about one thing:  What does AUTH_REQUIRED variable
do?  I am still under the impression that all of my  mail will be
rejected if I flip it.
