Thats probably because you are viewing it from a mailscanner perspective.

The maildrop example in that link is making a single atomic delivery
action straight from mta to mailbox via spamassassin filtering.
Mailscanner doesn't work this way, mailscanner listens to a queue
directory, processes messages in that directory and drops the filtered
messages into another queue directory for the mail delivery agent to
collect and deliver.

You could achieve something similiar with an add on component, but to
the best of my knowledge there is no solution for this out of the box.

You could hand off from maildrop to mailscanner easily enough, but
you'd need a component that would scan mailscanners output directory
and feed the messages back into maildrop for mailbox delivery. If you
want to do the mailscanning on another machine, then you're going to
need a socket based listener to prime maildrop with the scanned
messages as well as an app to scan mailscanners output and feed it
back to maildrop.

You won't get mailscanner working with couriers queue directories
because mailscanner hasn't been adapted for courier's queues, and
unlike postfix etc (which mailscanner does work with) courier splits a
queued message into two distinct files, so its not a simple
adaptation.

In summary, that guys instructions, in my opinion, won't work for
mailscanner, and  I doubt he's tried it!
