I used to use ClamCour: http://www.becrux.com/index.php?page=projects&name=clamcour but found that it stopped working with ClamAV around ClamAV 0.90 or so. ClamCour has the added advantage of rejecting the email at SMTP time and can scan authenticated user's mail if desired (e.g. outgoing messages). However, without an update to support new ClamAV versions, it's not real useful to me at least...
