We use an application form. I guess this puts is in a minority and possibly at odds with some of the other folks on this list. We use the application form to gather important baseline data about our members (for ongoing data collection/analysis), to assess their fit, to help us balance our membership, and also because it creates enthusiasm and a sense of cache around the space and participation. Though obviously, these reasons may not resonate with many of you...
