just updated/restarted my servers and as soon as someone joined I got: pure 
server could not find file .. to hash .. [ in server log (benign for now)]
 
I think players can play and this error doesn't seem to be something new.. but 
should I be worried?

I've never experienced that problem in the matchmaking menu. I've only 
experienced it in a server if I'm attempting to put up a vote to change the 
level.
I think what Calder is saying is that if he goes to start a lobby for 
competitive matchmaking, he can't scroll over to choose, for example, 
cs_assault, because it's too far to the right.

currently makes any server that has more than 8 or so maps hard to vote using 
the in-game mechanics, you just have to hope your map is randomly picked at the 
end.
