Can confirm. Still happening on a daily basis on servers with custom maps. This problem was happening in TF2 in early November. John Schoenick replied to the HLDS mailing list saying "I've narrowed this down to an old typo in the engine that was exacerbated by recent changes". On 11/18/2016, John made a post to the HLDS mailing list with an Optional TF2 update which included a fix for this issue. Since that update, I have yet to experience this issue once.
I can't imagine why the CS:GO devs are leaving this issue for so long when it's been made apparent by another Valve dev that it's a relatively simple fix. Yet they can pump out more weapon cases and a useless new map filled with problems before they can resolve simple outstanding issues that cause players to constantly be disconnected from games.
