Still happening. I've experienced it on 64, 85.3, and 128 tick. All with
correct rates.

For me it usually happens right after the server loads a map or reloads the
same map. I've only had it happen on custom maps, not on any official. But
like Darulo mentioned, the game/map is nowhere near it's entity limit.

Again, around the time this issue started (Gloves update, November 2016),
TF2 had the same issue and their dev John S. made a post stating it was an
old engine typo which they fixed rather quickly after it was noted. Should
be an easy fix for the CS:GO devs since another Valve development team
already knows the solution.
