We are developing a
volunteer confidentiality form here at Wells Branch for general
volunteers and for those people who help with the web site and such. Do
any of you use forms for either of these types of people already? What
has been your experience with this?

Confidentiality
The Right to Privacy Act guarantees that no confidential information
will be given to unauthorized persons without the individual�s or
district�s written consent. Information concerning clients acquired from
any source will be kept confidential and discussed only with appropriate
staff or Board member.
As a volunteer
for the Wells Branch Community Library, I agree that any information I
obtain will be held in the strictest of confidence. I will not divulge
any passwords, or other information gained in the course of my volunteer
work with the library. This includes patrons book selections, websites
visited, requests for information, survey data, etc.
