Rather than using 'global tonemap' as I had earlier tried I am now finding much 
better results through the use of the 'levels' module and then pulling the 
mid-tone down (to the left) as needed.

This appears to be providing the best extended shoulder range and does not 
incur the noise premium that I was previously seeing.

Possibly a standard setting in the 'zone system' would also be doable but 
would probably be more fussy when making individual image adjustments.
