Options:

since you are connecting directly to the db, just create function in any 
language you use (php,java) to populate a attachment table every time the 
user login, with the messages flagged as unreaded. Since it will scan only 
the newest entries, it will be always fast. How many emails can you receive 
from one login to another? fifty?
anyway, I think the problem is the way how you construct the message and 
decode the mime.
