Ok, this works. There used to be an orca-autostart.desktop file in this directory but it appeared useless. Looking at the file it seemed to only work if visual accessibility was enabled. Therefore I tried to enable this by going to applications> system tools> preferences >system settings. However, when I select the universal accessibility settings it crashes returning me to the desktop. The syslog shows this: Nov 19 19:40:05 debian-laptop kernel: [13342.036203] gnome-control-c[17683]: segfault at 0 ip b07831e2 sp bfe41c50 error 4 in libuniversal-access.so[b077e000+f000] It doesn't look particularly good to me. Anyway doing what you said works, but in the process I've found another issue - I can't use the universal accessibility settings. Thanks for your help, Jason.
