Thanks for the bug list. I found one for the freemedforms project

It can be easily corrected, but I don't really know how to send the patch. I
kept both sqlite and mysql because the sqlite is the fallback driver in case
the mysql is wrong.
Corrected package description is:
Package: freemedforms-libs
Architecture: any
Depends: ${shlibs:Depends}, ${misc:Depends}, libqt4-sql-sqlite (>= 4:4.7.0),
libqt4-sql-mysql (>= 4:4.7.0)
Can someone help me?
