The DECODE OFF option; what would it not catch (i.e. Spam) had the option been turned on? I understand Base64, but what kind of HTML decoding?
This only affects filters. With DECODE OFF, filters will work as they were designed to work (checking the body of the E-mail for a word/phrase). Without the DECODE OFF, v1.75 will decode the base64 and remove HTML codes (so a spammer with "se<B></B>x" will trigger a filter for the word "sex").
If you do not use filters, then the DECODE OFF option would save a bit of CPU time that wouldn't be needed.
