I figured out when the "X-Spam-Tests-Failed" header does not get added to e-mail messages.  It appears to happen when domains are setup to use a "per-domain configuration".  For example, under the Declude directory we have a subdirectory named pointshare.com, and in that directory we have a copy of the $default$.junkmail file.
 
Pointshare.com is actually not a local domain on the IMail server, but rather Declude runs its JunkMail filtering and virus scanning on all pointshare.com e-mail messages then relays the messages that pass all JunkMail tests and contain no viruses onto our Exchange server.
 
Although all the X-RBL-Warning are always there, and all messages meeting our JunkMail weight settings or that contain viruses are held, the pointshare.com e-mail messages that are actually delivered to our Exchange never contain the X-Spam-Tests-Failed header, but all other non "per-domain" configured e-mail messages do have the X-Spam-Tests-Failed header.
 
I use the Spam Review GUI to review held e-mail messages and the "X-Spam-Tests-Failed" header does not show up there either, so I just wanted to clarify that it is not the Exchange server that is the culprit here.
 
Any thoughts on how to resolve this?
