Any idea why this would happen?
Yes, I have a very good idea. It's sending "HELO" or "EHLO" in an SMTP transaction, followed by a made-up host name.

The log file or X-RBL-Warning: header should show you the hostname that the ISA Proxy claims to be. Most likely, there is a configuration option in ISA Proxy that lets you choose what the hostname should be.
