Can't the HELO contain both a FQDN and IP address?

No. The HELO/EHLO data can contain either a FQDN or a "domain literal" (such as a properly formatted IP), but not both. So "HELO example.com", "EHLO mail.example.com", "HELO [192.0.2.25]" are all OK, but "HELO 192.0.2.25" is not (not properly formatted), and "EHLO mail.example.com [192.0.2.25]" is not (too much information).
