I see the problem! I'm attaching the DITA files.
I tried to observe the Getting Started doc conventions. There are a couple of
items that aren't covered in those conventions: command-line commands (vs. SQL
ones) and URL connection attributes. For these I used codeph, but if you think
they should be in italic I can make that change. I still haven't figured out
the principle behind putting so many things in italics.
