The former.

The choices I make here will affect the GUI for it, BTW. Just think of how you'd configure the formatting for two different types of references:

1) A book, where primary sort key and first field is the creator name, and secondary would be "Anonymous"
2) A legal case, where primary sort key and first field is that record title (Doe v. Smith)


Currently my formatter has internal rules for handling this stuff, so that it's mostly handled implicitly (which makes things simpler). However, I got tripped up on example 2, so need to rework things.
