This change creates a cyclic dependency between couch_mrview and couch
which is ungood. Its not necessarily a show stopper but I'd really like to put
some thought into not having the changes code here aware of couch_mrview at
all. I understand that the previous state of things was awfully dirty so its a
bit hard to argue about how much worse this is. Given there's open questions
about the other PR I'll try and keep thinking on this for awhile.
