Regarding Trac, which I suggested as a solution in an earlier post, you might take a look at the first screenshot (linked from the bottom of the page) for the Trac Distributed Peer Review plugin, < http://trac-hacks.org/wiki/PeerReviewPlugin>. The example shown displays comments on source code, but should work for comments on documentation too. It seems to me that this plugin would resolve many of the issues we've been discussing by tying comments to particular lines of text, rather than just collecting them by chapter.
