Sorry for the delay--the sample set turned into more of a project than
I expected. Somehow, it always does ;-)
Anyway, here's my first complete set:

I intended for the data to be sensible but I've only just now got it
all into the database and have not tested anything, not even the entry
forms, but maybe this will be enough to get started.
The sample data includes some silly & familiar names (therapists "Anna
Recksiek" and "Frazer Crane"); the meds are all
silly/fictitious/OTC=non-prescription ("Focusyn", "Cordrazine"),
except I included the real medications mentioned on p109 in the text.
I thought it best to avoid looking too much like real medical records,
but you may feel the silliness clashes with the more serious tone of
the book. We can switch to purely random names (as the patient names
are already) quite easily, if you prefer.
No attribution is necessary--really--but if you want to include it, my
real, actual name is: Joe Smith.
