One system that Bugzilla has (Which we don't employ) is a system of
vouching for users
Basically when a new user comes along, all of their bugs are marked
UNCONFIRMED - unconfirmed bugs don't appear on bug queries by default,
but you do get bugmail, and you can query specifically on them.
When a user has filed some good bugs, any non-new-user can vouch for
them and from that point forward any bug they file will get marked as
NEW rather than UNCONFIRMED.
However, I it still requires people to sign up for a bugzilla
account/etc...
I still think the simple web form, which dumped into a database that
was browsable, would do the job. If anything we could add an extra
internal field, bug #, which means the feedback has been 'converted' to
a bug...
