This statement reflects my main concern about this topic:
A bug ist not a bug because it has not been found, but a bug is a
bug because someone did a mistake. So, instead of thinking about why
this bug was not found, it could be more helpful to think about the
reason why this bug was produced.
No doubt, testing is necessairy. But one cannot produce quality by
testing. High Quality is a result of *preventing* bugs.
Maybe some of us mix up Quality Assurance and Quality Control.
Wikipedia: "Whereas quality control emphasizes testing and blocking
the release of defective products, quality assurance is about
improving and stabilizing production and associated processes to
avoid or at least minimize issues that led to the defects in the
first place."
So our question is: Who is in charge of Quality Management? Quality
Management includes Quality Control, Quality Assurance and Quality
Improvement. It´s a holistic task that respects all factors that
influence the generation of defects. For example this includes human
and social factors, communication, documenatation and knowledge
management as well as the organization of the company/project and so on.
Again Wikipedia: "In recent times some themes have become more
significant including quality culture, the importance of knowledge
management, and the role of leadership in promoting and achieving
high quality. Disciplines like systems thinking are bringing more
holistic approaches to quality so that people, process and products
are considered together rather than independent factors in quality
management."
