No one blames anyone for making mistakes. (Somehow I feel misunderstood!) I would like to emphasize what Frank Schönheit stated the other day: "...we take way too little measures to prevent bugs, we always concentrate on finding and fixing them."

A holistic Quality Management is meant to create circumstances under wich errors are less likely to occur.

As long as no one can name anybody who is in charge of the project´s overall QM, the project indeed does have a general Quality Issue (see the subject).
