I think I' might have brought this up here before, but I've been using Mantis (http://mantisbt.sourceforge.net/)for a while. It's more of a bug tracker/reporter than it is a help desk system, but fits the bill as a trouble ticket system. Mantis is PHP and Mysql based and installs "easy as pie" on a SME box. Lot's of features, but not so big that it is un-useable.
