We only made the concurrencyremote change in 5.5 - in earlier versions
you'll need to override the template. I haven't looked at your contrib -
you'll have to vouch for how well it works :-)

If concurrencyremote is 0 and qmail has been restarted, this should not happen.

We cannot ship a patched version of qmail and haven't seen a reason to
do so. We don't use qmail-smtpd, but our qmail install is 100% qmail-1.03
