I think the difference between selected text in MS Word and a Thickbox instance is that people would expect (for the most part) to be able to mouseover selected text and have some options appear. (Usually from right-clicking, but enabled by mouseover anyway)
The new effect with the menu fading in for selected text in MS Word is very cool, but for Thickbox, without some visual cue for clickable options, people wouldn't know where to mouseover at all. A good compromise would be to put the mouseover points on the TB frame with very low opacity, but visible nonetheless... then when someone approaches the text/graphics with the mouse pointer, the mouseover points can become more distinct in direct proportion to the proximity of the mouse pointer.
