The formula I've used since 1994 or so is:

annual salary / 100 = daily rate

(divide by 8 to get hourly)

I've gone through two other more sophisticated techniques that
involved a lot more data.  I've also looked at market-based pricing.
The answers were all within 5% of the answer from above.
