what we do as part of scrum, which is simple, but seems effective, is that everyone writes down all the good things and the bad things they can think of on sticky notes. you stick all the good things on one side and the bad things on another as you get the notes. then at the end you review them all, and pick 3 bad things to work on improving for next time and talk briefly about what you can do to improve them.
note this is for a 1 month "sprint" that is part of an overall project, not the whole project, but I think it's a useful exercise that might be applicable. The whole team participates and it's over in an hour or less, with some useful action items (but not so many that nothing ever happens) this doesn't deal with stakeholder feedback, just the team doing the work. but if you can get stakeholders to show up, then it might work for them too. MT PS: I prefer to do the bad news first and end with the good news, so everyone leaves feeling happy.
