Hi Fred, In the company I work for, most of the prototyping job is made with flash and Photoshop. The process generally involves at least a graphic designer and a simulation designer. The simulation designer is responsible for analyzing the taskflow developed by the UI designers and determining the best way to implement it(which is then sent back for the UI designers for evaluation). After approved, the simulation designer sends a list of graphic assets that will be needed to the graphic designer and starts prototyping the application using dummy graphics that can be easily changed later by the final ones through the flash library.
To get prototyping "rapid" we make use of a custom component library that act like a set of Visio-stencils and if an interaction needs a component that is not there, we build it from the ground in an extensible way so it can be later used in other prototypes.
