Day phone: [ ] Night phone: [ ] Or even simpler: How can we reach you? Phone: [ ] What's the best time to reach you? [Morning (8-Noon EST), Afternoon (Noon-4pm EST), Evening (4pm-8pm EST)] Alternate Phone: [ ] Will knowing if it's a mobile, home or work phone affect the frequency or method of calling the person? We use a cell phone as our home phone, so I usually put that in the Home phone field, then wonder if I need to put it in the Cell phone field too. The method above is more naturally how I think about the 3-5 phones I have at any given time. Label input box they could label it whatever (e.g. Mobile, Home, Business) 2. A drop menu they could select from Mobile, Home, Business This makes sure you get a phone number and lets the customer/user select the appropriate label. Number 1 is more flexible, but depending on how you want it to report on your backend, you might select #2.
