I've had a similar problem in the past, where we wanted the user to
input as many phone numbers as they were comfortable with, plus an
email address. We had to have at least one of these to continue.

My solution was to have a section (e.g. a fieldset) called "Contact
details" with an instruction under the heading which says "You must
provide at least one of the following."

Then, the phone numbers we asked for were daytime hours, after hours
and mobile. We let people put any sort of number into any field (e.g.
daytime hours could be a mobile).

One other comment: Todd's "add field" pattern is nice, but not
accessible if you've got scripting turned off. Standard AJAX versus
accessibility problem really.
