We use an "Add field" pattern. Simply put, after the field, typically  
to the right, we place a [ + ] button. Selecting this button repeats  
the field below. Something like this:

|                       |  [ + ]
--------------

Selecting the [ + ] above would reveal a second field below...

transition

--------------
|                       |  [ - ]
--------------
--------------
|                       |  [ + ]
--------------

This continues down the page until the user stops, or you no longer  
provide a plus button (e.g. you want to limit the additional fields to  
a particular number like 3 or 5).
