My management has asked me to compile a list of well designed online forms (structurally and visually). In particular those that consist of a large number of fields (30+) and a number of different types of data being requested (Personal information, demographic information, preferences, and so on) If you know of anything publicly accessible or have any private examples you'd be willing to share, I'd greatly appreciate it.
