I thought there might be a few at the Australian Tax Office, but I
think they've taken to using custom software. Might be worth digging
around though: http://www.ato.gov.au
Or you might try and apply for Australian Citizenship online: https://www.ecom.immi.gov.au/citz/applyNow.do?form=1300t
(you can save the form and come back to it, so you don't need to
commit!).
They're pretty rubbish as forms go, at least in terms of look, but
they do output (or used to) a PDF copy just like the paper one. I
remember thinking it was remarkably easy given how hard it was to get
residency.
