Hi Adam A while back I designed two moderately-long online forms for the Australian utility, Origin Energy. They are the first two forms accessible from this page: http://www.originenergy.com.au/2070/Sign-up-online. (The third form was designed by Origin themselves adapting the design of the first two forms.)
Unfortunately some of the other large online forms I have designed are not publicly available or not built yet. But good general places to look are other energy retailers, banking sites (e.g. online loan applications) and health insurance sites. You might also want to ask your question on the mailing list of the international form professionals body, Business Forms Management Association.
