The newest version of iTunes seems to detect changes to the MP3 files and synchronises them onto the iPods automatically.
I have a FLAC library with embedded images, and use flac2mp3 to maintain a shadow library of MP3s since I have too many albums to be able to fit even a chunk of them on my iPods in lossless format, so when I change an image in a FLAC file it will be propogated into the MP3 file automatically. If I changed tags or images in the old days I'd have to delete the files from iTunes and then re-import them but I noticed when I built a new iTunes virtual machine a couple of weeks ago (to get rid of the performance hit of all the iTunes services running on my PCs all the time) that the new version automatically updates itself at startup. It's actually quite slick now, and I can now specify individual albums that I want synced onto my smaller iPods.
