Ah, I've just figured that you may have got your artwork into iTunes before you started embedding it into the MP3s. If that's the case then iTunes stores the artwork separately for those files, so it may take precedence over the embedded artwork.
I'd try deleting a couple of albums as a test, adding them back as MP3s and then changing the covers to see if it updates okay on the iPod.
