I checked when I first saw it, and I can confirm that is was in use, at least informally, before Nov 2016. Here is a short blog it inspired when I saw it earlier: https://healthyalgorithms.com/2016/08/10/software-carpentry-training-and-the-code-of-conduct/

We worked on formulating the text of the CoC and the enforcement manual during that fall. I think it went live sometime late November/December of that year. However, this is the formal CoC. I think we have had an informal one in force from way back in the day. Maybe others can expand on that? Karin (FYI, I'm the current Chair of the CoC committee). _______________________________________________ Discuss mailing list Discuss@lists.software-carpentry.org http://lists.software-carpentry.org/listinfo/discuss _______________________________________________ Discuss mailing list Discuss@lists.software-carpentry.org http://lists.software-carpentry.org/listinfo/discuss
