My memory in dealing with Spoken Interface and panther, was that more
often than not if there was a mouse it was that Spoken Interface
would crash. In fact I remember it crashing a fair amount in certain
Aps like Safari for example.
Couldn't do a thing with I tunes, except open it and read "scroll
bar." Nothing else that I can remember. Had me wishing for a while
about going back to Outspoken, then it timed out.
Apple still needs to do some work on this pertaining the mouse, or
there is something different with some Aps that I don't figure out.
I also remember Spoken Interface wasn't as friendly about multipal
options for interaction as VO is now. For mail for example the most
stable was to do Command A or whatever the combination was and just
have it read the whole document. Kind of a hastle with many lists.
There have been deffinite improvements, stability being the major
one, however, we also have a way to go yet.
But I also have a feeling there will not be a major upgrade with VO
until the next major OS is made, the next cat in other words unless
they totally want to prove me wrong. They are welcome to do just that.

Voice over was most definitely tested by users. I met several of
them when I was at the acb convention in Alabama in 2004. They all
had issues, though I don't recall anything about mouse keys coming
up in conversations I'd had with them at the time.
There are a couple here on this list, but if they choose not to
speak up, that's their choice. But, I can assure you Apple did
test this with users, and did not keep testing in house.
