Here are the fields I see for the database

Program name
Program Version
Program Developer
Program Website
VoiceOver compatibility on a scale of 1 to 5 with 1 being not compatible and 5 being completely compatible.
Program notes
Program tips
Date of addition to database
DB record number (needed for programing use)
ISBN number of software package (if known)
