you can add an action to the sign in event that gets VO to read who came online in prefs. Just find the events table, pick the event you want to add too, and remember that VO-h reads a lot of those unlabeled buttons. The one to add an action is just to the write of the table.

The tone which goes up means a person has just signed in, and the other
sound means someone signs out. You can change this in the preffences if you
want to.
The "system has new dialog" means someone has signed in too. I don't know
why VO is unable to read who has signed in, but I hope that will be fixed
soon.

This is a long shot, but I have a feeling hwat's happening, is a contact is comeing online, or going offline, not totaly sure, mine is saying the same thing, and i have no idea to get at whatever it is that may be the new dialog. I find when i have Adium up, that it's also happening when somebody signs into Skype as well. Could that be something to do with Growl. I have a feeling that's what it is, but I'm not sure.
