I've seen this occasionally if you've opened a dialog box, such as open or save. Once the dialog closes Voiceover can stop reading. It doesn't happen often enough for me to find a pattern, but it always happens after a dialog closes. The only way I've found to make it work agai is to turn voiceover off and back on. Another possibility is that you have a window covering the text area, try hiding all other apps with command-option-h. Also, make sure cursor tracking is on, in particular make sure the keyboard and Voiceover cursors are tracking one another. If they aren't you may not actually be focused in the edit area though the VO cursor is there.
