Hi Scott,
Try using VO-Shift-W to have VoiceOver read everything in the iTunes
window once you've opened the application. This should announce all
the buttons at the top, then go through the entries in your sources
table and songs list if they're displayed. Most likely you need to
interact with the the sources table or songs table to have VoiceOver
read them. If you've just started up and tried to connect to the
iTunes Store to set up an account, there may be a membership signup
dialog window up. Try taking a look at Tim Kilburn's description of
the layout of the iTunes application window at:

It hasn't been updated for iTunes 8, but it's a pretty good overview
of the general layout. The menu structure has changed a little bit,
and the iTunes Store became totally accessible, but this is a good
starting point if this is your first encounter with iTunes. Mike
Arrigo also did a recent podcast including iTunes for Blind Cool Tech,
and there are some older Screenless Switchers podcasts on the subject.
Let us know what happens when you read out the iTunes window. It's
possible to not have VoiceOver view the control menus in iTunes if
you're in the middle of playing a video podcast or movie -- in that
case the image fills the window and you need to press escape to leave
the video display mode. But you should be able to diagnose things by
having VoiceOver read your iTunes window and announce, "Sources Table,
Library, Music, Moves, TV Shows, etc.") after reading all the buttons
at the top of the window.
