What is your From address in cfmail? If this address is not a valid email address it general won't cross mail servers. It will work on your local network most likely. But when it transmits outside of your network, foreign mail servers will be looking for a valid mxrecord to verify it's coming from a valid source before they allow it. All mail servers do this these days because of Spam.
This is at least 1 thing to look at. I've inherited apps as well and this was my problem. After I changed to a valid email address (that can be sent to from the outside) everything worked. This also allows you to get rejects if something is rejecting the mail.
