The semantics of the above chapter elements are not crystal clear.

Instead, I would have written

<chapter href="../ditamaps/Contact_Contacts.en.ditamap" format="ditamap"/>

which would be slightly better.

I didn't find there anything related to the problem you describe.

* your bookmap contains frontmatter/booklists/toc or

--> May be you are talking about the ``PDF toc'' (AKA PDF bookmarks)? If
this is the case, pass XSLT stylesheet parameter "-p pdf-outline yes" to
ditac.
