I experienced this problem before. There is a solution, hopefully it will be the same in your case.
You must have multiple copies of a file name in the same folder. Though they may have different extensions (.wav, .rcy, .rex, .sfk, ect ect) the program gets confused as to which file to import. Say you worked on a sample in Sound Forge, you & save it as a .wav, but what you may or may not know, is it saves another file with the extention .sfk. Then if you work on the sample again in Recycle, sometimes you save a .rcy & a .rex with the same file name. You get what I'm saying? Basically just delete all those dodgy files you don't need, or save them in another folder. I've not only experienced this with Reason, but Cubase as well. When you get into a project, you tend to accumulate alot of useless files.
